These Terms of Sale set out the terms and conditions on which physiotherapy services are provided by Us to Consumers.
Please read these Terms carefully before booking an appointment. By making a Booking, You agree to be bound by these Terms.
All information We provide to You, whether on Our website, by telephone, or in person, forms part of the contract between You and Us where You rely on it when deciding to book or attend Physiotherapy Services.
These Terms apply only to face-to-face physiotherapy services. Terms governing use of Our website are separate and available on Our Site under “Website Terms of Use”.
In these Terms:
"Booking
means a reservation made by You for face-to-face physiotherapy services, whether for a single appointment or a block of appointments, booked online or by telephone.
"Booking Confirmation"
means Our confirmation of Your Booking, provided verbally or by email.
"Consumer / You / Your"
means an individual patient booking Physiotherapy Services for personal use and not for business purposes.
“Contract”
means the legally binding agreement between You and Us for the provision of Physiotherapy Services, formed when We issue a Booking Confirmation.
“Physiotherapy Services”
means face-to-face physiotherapy services provided at Our clinical premises, including assessment, examination, diagnosis, advice, treatment, rehabilitation, and exercise prescription.
“We / Us / Our”
means Prince Haastrup, trading as PH Sports Rehab. Information About Us
Our Site, phsports.rehab, is owned and operated by Prince Haastrup, trading as PH Sports Rehab.
Mem's Weightlifting Club,We are registered with the Health and Care Professions Council (HCPC). We are a member of the Chartered Society of Physiotherapy (CSP). We are not a limited company.
Consumers Only and Age Restriction
Only Consumers aged 18 years or over may book Physiotherapy Services. These Terms do not apply to business-to-business services.
Bookings, Pricing and Availability
Physiotherapy Services are provided by appointment only. Bookings may be made online via Our Site or by telephone. Availability is subject to clinician availability. All prices are communicated before Booking and must be agreed before services are provided.
How the Contract Is Formed
When You make a Booking, You are making an oƯer to purchase Physiotherapy Services. A Contract is formed only when We confirm Your Booking. Your Booking Confirmation will include:
Payment
Payment for Physiotherapy Services must be made either in advance or at the time of the appointment unless otherwise agreed.
We accept payment methods as stated on Our Site or advised at the time of Booking.
If payment is not received, We reserve the right to cancel the aƯected Booking. Where payment remains outstanding, We may also suspend or cancel future bookings until payment has been received. This does not aƯect Your statutory consumer rights.
Cancellations, Rescheduling and Missed Appointments
If You need to cancel or reschedule an appointment, You must give at least 48 hours’ notice.
If You cancel with less than 48 hours’ notice or fail to attend, We reserve the right to charge the full appointment fee, reflecting the time reserved and the likelihood that the appointment cannot be reallocated.
We may, at Our discretion, waive cancellation fees in exceptional circumstances.
Your Right to Cancel (Consumer Law)
Under the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013, You have the right to cancel a Booking within 14 days of confirmation.
However, if You request that Physiotherapy Services begin within that 14-day period, You acknowledge that:
Provision of Physiotherapy Services
Physiotherapy Services will be provided with reasonable care and skill in accordance with professional standards.
We may, where necessary, substitute another suitably qualified physiotherapist. We do not guarantee any specific outcome. Results vary depending on individual circumstances.
Health, Safety and Fitness
By attending Physiotherapy Services, You confirm that:
If You have concerns about Your health, You should seek advice from Your GP or appropriate healthcare professional before attending.
Our Liability
Nothing in these Terms excludes or limits liability for death or personal injury caused by negligence, fraud, or any liability that cannot be excluded by law.
We will not be liable for indirect or consequential losses, including loss of income or business.

If You have a complaint, please contact Us as soon as possible so We can attempt to resolve the matter.

If a complaint cannot be resolved, You may be entitled to refer it to an Alternative Dispute Resolution provider. Further information is available from Citizens Advice.
Data Protection
We use personal data in accordance with Our Privacy Notice, available on Our Site. Other Important Terms
We may transfer Our rights and obligations if We sell or transfer Our business. Your rights will not be aƯected.
The Contract is between You and Us. No third party has rights to enforce it.
If any part of these Terms is found unenforceable, the remaining provisions remain in eƯect.
Law and Jurisdiction
These Terms and any dispute arising from them are governed by the law of England and Wales.
These Terms of Sale apply to all bookings made on or after 12 March 2026.
